Error Reporting

The implementation of an error reporting system in an organization can identify local system hazards, can be used to share lessons across organizations and can aggregate experiences for uncommon conditions (e.g. EMS giving a drug to a pediatric patient), but most importantly it can be a method to change patient safety culture. The error reporting system can be used for adults as well as patients.

Reporting EMS personnel will be given the option to make a follow up phone call to the study staff to discuss what they reported. After completing the report, the individual will be given a unique number that will indicate that they have reported the error or concern.

It is essential for all parties to know that there will be NO disciplinary action for self-reporting.
